In 2024, the College continued to work with the OAO and the University of Waterloo School of Optometry and Vision Science, while engaging with the provincial government, to advance scope modernization. Many optometrists are already trained in these areas, and updating the scope will enhance public access to care. Our discussions with all parties have been constructive, and we are encouraged by the progress to date.

We remain committed to “right-touch regulation,” applying the minimum necessary force to achieve the greatest public benefit. This ensures concerns are addressed seriously, fairly, and always with the public interest at the forefront. We also prioritize open communication and transparency, providing members with the information and resources they need to meet their obligations with confidence.

The College continues to adapt to advances in clinical care and technology so our standards reflect current best practices and emerging evidence. Financial stewardship remains strong: despite inflation, College fees have not increased in over a decade thanks to careful management and an innovative team.

We value our collaborative relationships with professional associations, educators, and government, working together to advance patient care and public trust. We also acknowledge the role Ontario’s optometrists play in delivering safe, effective, and ethical care.

Emerging research suggests that physical activity may help protect patients from visual field loss, even after accounting for disease severity. Higher levels of walking and moderate to vigorous exercise have been associated with slower glaucoma progression in both observational and longitudinal studies.

Physical activity may benefit patients with glaucoma through multiple mechanisms. Regular aerobic exercise appears to reduce IOP by improving aqueous humor outflow and enhancing ocular perfusion pressure. Moreover, regular activity supports systemic health, helping to prevent conditions like diabetes and hypertension that can worsen ocular outcomes.

Beyond IOP, physical activity has been associated with better outcomes in other neurodegenerative diseases like Alzheimer’s and vascular dementia. Physical activity may exert direct neuroprotective effects. Both animal and human studies have linked exercise to increased levels of brain-derived neurotrophic factor (BDNF), a protein essential for neuron survival. Given that glaucoma involves the degeneration of retinal ganglion cells, enhancing BDNF may provide meaningful protection.

While exercise will not replace traditional treatments, clinicians should view physical activity as a legitimate adjunctive strategy. Encouraging patients to move more may help preserve vision and improve their overall health in the process. A summary of our work at the College during the first year of my presidency:

1-Worked through the Tripartite Working Group (alongside @uwoptometry and @ontariooptometrists ) to provide the government with recommendations on scope expansion in the public interest.

2-Continued commitment to public protection through “right touch regulation” (the minimum regulatory force needed for maximum positive outcome).

3- Achieved a balanced budget while membership dues have not been increased in over a decade (despite increased costs of operation and Inflation).

4-Committees (with invaluable staff support) worked diligently to update regulations, improve bylaws, and to execute the core mandate areas of the College.

While NAION is the second-most common optic nerve condition, Eltis says it’s a “far distant second” to glaucoma. If you are eagerly awaiting the total solar eclipse on April 8, there are some precautions you will want to take to protect your eyesight.

Dr. Mark Eltis, a Toronto-based optometrist, told Global News that viewers of the eclipse will need special glasses, with ISO 12312-2 rating, to protect their eyes from any possible damage.

“Those glasses are much darker than regular sunglasses,” he said. “You can’t wear regular sunglasses.”

The eclipse can provide a false sense of safety since the sun will be dimmer, but harmful rays are still present, he added.

Without proper eye protection, the light rays that do shine through will travel to the back of the eye, called the retina. That’s where the cells interpret the light and send the information to the brain. Damage from those rays can happen within a minute, Eltis said, and you won’t even feel it is happening.“Our pupils will be more dilated because there’s less light, so the rays will go to the back of the eye and they can burn those cells that interpret the light,” Eltis said. “That’s the danger.”If you are watching a total solar eclipse, as will occur in certain parts of Canada, Eltis said it is possible to look at it without special glasses when it is in totality, but said it is dangerous if you are not sure when exactly that is happening.

The eclipse will pass through Ontario, Quebec, New Brunswick, Prince Edward Island, Nova Scotia, and Newfoundland and Labrador. Several cities and towns will go into complete darkness for a few minutes during the day. Even if you wear the protective glasses, Eltis suggests you look down at the ground before looking up at the sun, and then look down again after viewing the eclipse.Symptoms of eye damage include blurry vision as well as black spots. The next frontier in glaucoma prevention lies in tailored and natural strategies. Lifestyle modifications, including nutrition, will play a much larger role in the optometrist’s game plan. The finding that niacin intake may decrease glaucoma risk in women (but not in men) is a perfect illustration of such an intersection of individualized risk assessment and prevention. A holistic approach to glaucoma care is not simply a slogan but is starting to be backed by the literature. That having been said, the public should be very cautious about being led into a false sense of security that supplements alone can inoculate them from glaucoma risk.

As with any therapy, experts must be consulted, and data must be studied carefully. Proper context should also be provided. Patients on glaucoma medication or requiring surgery can’t simply discontinue treatment and take vitamins. It’s allergy season again! If your eyes are super itchy especially at the corners closest to your nose you may be suffering from allergic conjunctivitis. Some patients get it worse in the spring and some suffer more in the fall. It depends on wether you are sensitive to ragweed, pollen or something else. See your eye care provider if you have any symptoms to get a proper diagnosis. Cold compresses are the easiest non-drug therapy. Some over the counter drops and natural remedies can be effective for mild symptoms. For prescription medication, drops are more effective for eye symptoms and generally make more sense over oral treatments. I’m using Bepreve which has the added benefit of potentially helping with nasal congestion as well. It is an antihistamine and mast cell stabilizer. This class of drugs is generally safe and effective with few serious side effects. For best results, the drops must be used (without interruption) twice a day for at least 14 days. They won’t work as well if you just put them in once in a while when you feel the itch. (You only get the temporary relief of the antihistamine without the shutting down of the allergic chain reaction that the mast cell stabilizer provides). I generally suggest they be used as soon as allergy season starts until it ends. It will take a few days for it to really kick in so don’t wait for the itch to get out of control. If you’re experiencing significant redness or inflammation you’ll need additional therapy as this type of drug primarily targets itching. If you have undiagnosed or untreated dry eyes or sun damage, allergies can be far more irritating. The most important thing is to see your eye care provider right away. Do you get eye allergies? The strength of the structure-function relationship is known to be weaker in the early stage of glaucoma, reaching its peak in the moderate stage. In advanced glaucoma, structural measures such as OCT have limited value because of a floor effect, while the results of visual fields are unreliable and inconsistent, making the true extent of damage difficult to determine. The structure-function relationship difference between NTG and POAG remains poorly understood, with some studies finding no significant variation. However, most studies focus primarily on early-to-moderate glaucoma.

In this study, Sung and colleagues found that in advanced glaucoma POAG patients had more diffusely distributed VF defects, while NTG sufferers had more severe VF defects at the superior nasal quadrant. However, macular RNFL thickness in NTG eyes showed a better-structure function relationship.

The true connection between NTG and POAG remains controversial, but they are clearly related conditions. Glaucoma of all types can follow a variable course, and we must diligently monitor each patient carefully without pretending to know individual outcomes. Future research will provide greater insight into glaucoma staging, but listening to a patient’s lived experience is particularly critical, as clinical tests are more likely to fail with advanced disease.

Glaucoma patients can and do have central defects early in the disease process. This type of damage has a greater impact on quality of life and should therefore be vigilantly guarded against. How do we identify those patients at risk for early central vision loss before structural or functional damage is evident? Structurally, inferior temporal retinal nerve fiber layer/ganglion cell complex loss is the clearest indicator corresponding functionally to superior central visual field loss. Research from UC San Diego suggests that corneal hysteresis (CH) may be an “early warning” biomechanical marker for central vision loss risk much like pachymetry is employed to highlight glaucoma risk.

Previous studies found that low CH was a predictor of faster retinal nerve fiber layer decline and global VF progression. The cornea’s ability to dampen pressure (CH) may also be a reflection of the optic nerve’s capacity to do the same. Such inherent risk assessment could be a further qualifier to true measurement of transcorneal pressure. This study by Kamalipour and colleagues showing low CH was modestly associated with earlier and more profound central loss provides further justification for biomechanical measurements to enter the mainstream. It’s one of my favorite things to do because it combines the art and science of optometry. It will be my first time on a plane in over 2 years! Becoming a Fellow was one of the most rewarding experiences of my career. It is a voluntary process taken on by about ten percent of American optometrists and a far smaller percentage in Canada. While some see it as a qualification reserved for those in academia, I found it to be extraordinarily clinically useful. It is not a mere theoretical pledge to lifetime learning. The fellowship process can include writing case reports, obtaining advanced training (residency)or degrees, publishing papers, leadership activity and passing a rigorous oral exam tailored to your particular contributions. It makes you realize school only scratched the surface of most topics and that information in healthcare changes at light speed. It’s a wake up call and a humbling experience. Equally important as acquiring knowledge is realizing the vast amount of information you likely don’t know. It would take several lifetimes to become a specialist in all aspects of the field. The goal is enhancing your clinical skills in certain areas and knowing when to send to expert colleagues in others.
